An approved design and a production-ready design are not the same thing. The gap between them is where packaging consistency is usually lost.
A design gets signed off once. It then needs to work across a dozen pack sizes, several markets, and more than one production method. Each version is a small decision on its own. Text that fit one size needs to be resized for another. A colour that worked on one material can read differently on the next. Individually, none of these choices is significant enough to raise a flag. That is why they go unnoticed until several versions are already in the market.
This is a graphic design problem before it becomes a cost problem. Catching a drift in one pack size after print is far more expensive than catching it before the file leaves the design stage. Treating each of these as a separate task, rather than a single design applied consistently, is usually where a range starts to look less coordinated than it was approved to be.
